A large group from the @EosinophilSoc including @EosDoc @adklion @TPongdee and others published retrospective data on biologic use in #HES. Covered during the last @AIMedEdJC #AIMedEdJC

๐โฌ๏ธ interest in using biologics for patients with eosinophilic disorders
One main issue: โผ๏ธ Most not approved or have warnings for #eosinophil associated diseases like HES

@somewheresy @DrCorinneHappel @TitanOfTryptase Thanks for sharing your experience with us. Hearing firsthand accounts of how a person was diagnosed is a helpful way for all of us to learn. #AIMedEdJC
The effects of corticosteroids can linger for a few weeks. If an eosinophilic disorder is suspected & it is safe to stop corticosteroids, an eosinophil count can be checked again after a few weeks to see if there is an elevated baseline level.

Thanks @somewheresy for the great teaching point!
Eosinophil counts quickly drop (often to zero) after oral/iv corticosteroids. Do NOT use a normal eosinophil count to rule out eosinophilic disease without verifying a person was not on corticosteroids.

Thanks @JACIInPractice for providing open access to this month's #AIMedEdJC discussion article (link below)!

@karenrobbinsmd @winder_gill @AIMedEdJC @ckeet @aolayink Thanks for sharing this link, @karenrobbinsmd! My mind was thinking the same last night and am glad you posted the article.

Thanks for bringing this up, @karenrobbinsmd and thanks for the initial post, @winder_gill!
Great article to take a look at๐ relevant to this weekโs #AIMedEdJC topic
@winder_gill @AIMedEdJC I think thereโs other data out there to support thisโฆ.am recalling a @ckeet @aolayink article where amongst publicly insured individuals, โWhite patients accounted for a disproportionate amount of biologic treatmentโ #AIMedEdJC

@AIMedEdJC I think there is a larger 'takehome' message not just for us, but for #primarycare and those involved with our patients - We often see eosinophilia on labwork, and yes, it can be associated with atopic disorders, but please consider the differentials! #AIMedEdJc @AIMedEdJC

Do you appreciate @AIMedEdJC journal clubs?
Would you like to get involved in the planning process or to lead a future journal club?
We welcome fresh energy & ideas!
Please reach out to @DrCorinneHappel for more info.

Particular thanks to author @KhouryMD for joining in tonight's discussion! We really appreciated having your expertise.

@shaunah07 @PAallergy @AIMedEdJC If you can make the eos go down some with skin care...then more likely AD associated eosinophilia. If no matter what you do to the skin eos remain elevated and refractory skin dz then probably other way around. No easy answer but clinical judgement #AIMedEdJC
@TitanOfTryptase @AIMedEdJC I have had this same thought @TitanOfTryptase -- I sometimes wonder whether we don't get a high enough eos count for EGPA criteria in some patients because we can't safely wean off of steroids due to severe asthma. #AIMedEdJC
@AIMedEdJC Correct. Although off-label approvals will work for some, not all will have that privilege depending on insurance (or lack thereof). If data from a Ph3 trial supports efficacy, it will be beneficial to many more patients with a completed trial. #AIMedEdJC
Great reminder that there are ongoing HES clinical trials in which patients may qualify for enrollment.
#AIMedEdJC
@TitanOfTryptase @AIMedEdJC A8: also when there's an active trial in a #raredisease you can get more data and support potential future approval by referring to ongoing trials: https://t.co/LLIL70eEBF

We're privileged to have eosinophil experts join in many of the @AIMedEdJC journal clubs! Any additional take-home points they or others would like to leave from tonight's article?
#AIMedEdJC
@AIMedEdJC Philosophical question: in pt a AD w elevated eos do they have AD w elevated AEC as part of the disease or do they have HES manifesting as AD? #AIMedEdJC
@KhouryMD @AIMedEdJC Perhaps better put: It sometimes seems random what is approved by insurance companies and what isn't. #AIMedEdJC
