71.8% of women in my survey reported brain fog. Only 24.9% reported hot flashes. So why does menopause research still center almost entirely on the symptom fewer women actually have?
I’m seeing that the latest fear in menopause medicine is no longer “Should I take hormones?”
It has become:
“Am I taking enough? Is my level high enough? Did I pick the perfect estrogen? Am I missing the perfect protocol?”
And here’s the truth: the science is not that precise yet.
Yet, good menopause medicine is not about chasing perfection. It is about thoughtful adjustments, symptom improvement, and personalization over pure optimization.
New survey data from 766 midlife women:
~55% of women on hormone therapy are using BOTH systemic and vaginal estrogen.
Primary care surveys typically show 15-25%.
The gap is not about what's appropriate. It's about what's being prescribed.
Women going to specialty menopause care eventually get both. The ones who don't? Often stuck with incomplete treatment.

If you feel sedated on progesterone, that's because it interacts with brain receptors tied to calm and sleep. For some, it's a huge benefit. For others, it leads to a burden of side effects, which I call "progesterone intolerance." What side are you on?

And when they finally find a clinician who listens and understands — life changes. Would you, or have you, invested in your healthcare?

The FDA’s decision to remove the boxed warning from many menopause hormone therapies is sparking overdue conversations. I shared my clinical perspective with USA Today on what this means for women in midlife. Read the full piece: https://t.co/kxK0ULPnm9
The FDA just removed the black box warning on hormone therapy. Twenty-three years of fear and misinformation, finally being corrected. About time we treated women’s health with science, not stigma.
Thanks to @DrMakaryFDA and the @US_FDA for changing the trajectory of midlife women's health care today, and into the future. Truly November 10th, 2025 marks a historic day for women seeking care for perimenopause and menopause
Getting better information out on HRT is long overdue. 50-70M+ women over the last few decades have been steered away from potentially life-changing therapy due to fear and medical dogma.
