Doctors in training are encouraged to consult with senior colleagues when uncertainties arise.

However, resident doctors may face reprimand for excessive contact with senior staff.

This dynamic often stems from an environment of apprehension and detachment fostered by some senior doctors, creating a challenging cultural reality within medicine that is not adequately addressed.

From my perspective, a senior physician who is consistently inaccessible to resident physicians' inquiries should not be entrusted with supervisory responsibilities over them.

The IVC is useless at:
- telling you what to eat for dinner
- acting as a fuel gauge for volume status
- predicting fluid responsiveness

HOWEVER.

The IVC is incredibly useful in helping us decide on the optimal fluid and decongestion strategy for your patients.

The diagnosis of “UTI” is dangerous.

Question it and scrutinize it.

In this case, the hyper acute onset of AF RVR in the setting of isolated pyuria was in fact NOT a UTI.

It was bilateral PE.

UTI is a diagnosis of exclusion (and I could say the same for “CAP”).

Yesterday, I posted a poll showing Hb 10.4 and Hct 41 and asked whether or not the patient had anemia.

وعمومًا أغلب أطراف المعركة هم من التخصصات ��للي اللايف ستايل حقها متعارف عليه إنه مريح أكثر من غيره
أما إحنا حقين الباطنة وزملائنا في الجراحة العامة مثلًا ماشاركنا المعركة ولا نفكر ندخلها 😂
عارفين إنه تخصصاتنا متعبة وممكن تأثر في نفسية الواحد بس دخلناها بإرادتنا وحابينها عادي 🤷🏻‍♀️
اللي يحب ال��اطنة يدخلها واللي مايحبها يدخل شي غيره مو لازم أنا اقنعكم بتخصصي ولا لازم إنتم تبرروا ليش دخلتم تخصصكم
في الأخير المجتمع محتاجنا كلنا بكل تخصصاتنا ولو الكل اتجه على تخصص معين لمميزاته مين بيعالج المرضى اللي يحتاجوا باقي التخصصات؟

فيه شخص معي بروتيشن عليه حركات قليلة ادب متغطرس و متكبر يعني انت جاي تتعلم وتذم في التخصص اللي تتدرب فيه وتقول انه مايضيف لك شيء وانت اصلا متعلمها وماتحتاجها لكن في نفس الوقت تسابق على الـProcedure وتحاول تسوي كل شيء وتخرب على زملاءك فرصة التدريب ، استغفرلله بس !!
