Isotretinoin is a powerful oral retinoid (vitamin A derivative) mainly used for severe, treatment-resistant acne — especially nodulocystic or conglobate acne that hasn’t improved with antibiotics or topical therapy.

🔹 Main Indications

Severe nodulocystic acne

Acne with scarring or high risk of scarring

Acne causing significant psychological distress

Recalcitrant acne (failed ≥2 other therapies)

🔹 Mechanism of Action

It targets all major pathogenic factors of acne:

↓ Sebum production (shrinks sebaceous glands)

↓ P. acnes proliferation

↓ Inflammation

↓ Keratinization of follicles

🔹 Major Side Effects (high-yield for exams)

Teratogenicity (strict contraception required)

Dryness of lips, eyes, and skin (cheilitis, xerosis)

↑ Triglycerides and cholesterol

↑ Liver enzymes

Depression or mood changes (rare but monitored)

🔹 Key Notes

Pregnancy is absolute contraindication → mandatory iPLEDGE program (in the U.S.).
