When activation lies: three flutter circuits in a single redo procedure.
A 63-year-old patient with recurrent atypical atrial flutter after multiple prior ablations. Three different flutter circuits were induced, with activation patterns that could easily mislead. The case highlights why activation mapping alone is never enough—integrating LAT histogram, propagation, voltage and entrainment was essential to identify the critical isthmus and achieve sinus rhythm.
