For the analytics, this sounds more related to cookies than an opt-in form checkbox.Check out the "GDPR Cookie Consent" plugin from the WP repository which is free and might help

GDPR does not allow you to have required checkboxes as you cannot make access to your lead magnet/service conditional on someone giving consent.
