Clinical Nurse Specialist @ NHS -All views are my own and do not represent those of my employer/regulator. Repost/reply/view does not always imply endorsement
I care deeply about my patients. I get intensely frustrated when they can’t get what they need - from us or other services. I despair when other services deflect their responsibilities onto me. I own our service’s inadequacies, but other services need to do the same. If the NHS can’t live up to expectations, say so. This is a govt issue. If you can’t do something or aren’t allowed - explain. Don’t deflect to the GP - they don’t make the rules. Hold the right people accountable, follow up on your own responsibilities. GPs are not community juniors/admin.
Fascinatingly frank chat with Nicola Ranger from @theRCN. She openly admitted chief nurses and @theRCN itself has been too weak on nursing. Worth reading in full:
@Keir_Starmer Who’s life is the NHS expected to contort itself into magical shapes for? Patients? Staff? - it does neither, and nor should it. A taxpayer funded healthcare system cannot be expected to provide its core functions when it’s so woefully mismanaged - starting with your government
@mgbmarkb@TheEMboardround And F1s/Newly qualified nurses can’t be expected to come out of medical/nursing school full of exposure or situational awareness either, that’s where as seniors we have to step up and take every opportunity to help them in gaining said experience
@mgbmarkb@TheEMboardround I had a similar experience when I was a staff nurse, and well remember that feeling of being told I was overstepping. Your grade shouldn’t mean you don’t do everything you can to get that patient the best care and help your colleague
@TheEMboardround In support of this F1 - this also reflects on the nursing staff - this is exactly the sort of situation where nurses need to make a difference and help that doctor to feel it’s right to request that senior input early on as opposed to 5 litres later
Thank u @DrTedros for highlighting again that there is no #healthcare without #nurses and #healthworkers. Making #healthforall a reality requires all countries to have a funded & actionable national plan and WHO must continue to have a Chief Nurse and robust workforce function.
Looks like Wes Streeting, Kier Starmer & the whole DHSC media spin team are bang to rights here by JVT. Currently no super flu & no NHS wide crisis. A lot of apologies needed from those players and the media in general. As we’ve said, even if it became a problem we’d use derogation.
I’d love to provide better care to patients
Virtually impossible when primary care decimated, mental health care decimated, social care decimated
Fewer hospital beds, doctors, nurses, scanners than any comparable OECD nation.
All these areas in the #NHS are overwhelmed & either at capacity or over capacity
Yet @wesstreeting & @NHSEngland raise expectations without raising the funding to support this
To resolve the ‘Magic Roundabout’ for patients fund more GPs, hospital beds etc
Blame MPs not GPs
Spot on. It’s one of the extraordinary own goals of my time in medicine that this has happened. With some consultation of resident and specialist doctors, the PA grade could have been made into the most gratefully-received role ever to be embraced by the medical community.
“What jobs do you guys do that don’t require medical training? And can someone else do them”? would have been ana amazing starting point for discussion.
Instead senior docs and NHS managers chose “Look, you’ll be basically doctors, and in couple of years we’ll get you on the paeds liver rota”.
This example from the NHS should be taught as a cautionary tale on every change management course in the world.
@DHSCgovuk@wesstreeting@wesstreeting tries and fails to play staff against staff. Doctors deserve pay restoration as do all healthcare professionals in the NHS, Maybe the government should change their approach?
The trust told staff in a message that it was limiting the use of registered nurses on the bank in certain areas of its hospitals, and nurses wanting to work bank shifts in these areas could pick them up at band 3 level instead.

@NurseStandard Absolutely ridiculous and so unsafe for registrants!- if this isn’t a red flag what actually is? @DHSCgovuk Stand up for nursing or look away the choice is yours
£62 million could have funded 500 full-time nurses for 2 years in critically understaffed NHS units.
Instead? It was spent on training Physician Associates and Anaesthesia Associates.
Priorities…
