Lung adenocarcinoma, 1-5% may harbor ALK mutation, mainly EML4 fusion on chromosome 2. ALK IHC is FDA approved as a stand alone test, poorly diff adenocarcinoma with signet ring cells and plasmacytoid features, intracytoplasmic PMN, cytopathology #168 #CytoPro
IMO, this cell would be unusual for a reactive lymph.
Downey Type II reactive lymphs are not this large and have moderately clumped chromatin (not blast-like).
The largest reactive lymphs (immunoblasts, Downey Type III) have coarse chromatin and basophilic cytoplasm.
Learning Points:
1. Mixed desmoplastic melanoma can result in pure spindle cell lymph node metastasis
2. Do SOX10 on lymph node to catch these sneaky mets!
