Experienced surgeons are often critical of modern residency training, and question the competence of new graduates.

I've been hearing about a competency crisis ever since the 2003 work hour rules, but it's now 23 years later and the sky hasn't fallen yet.

A simple question for SurgTwitter:
As incompetent and underprepared surgeons have made up more and more of the surgical workforce over the last 20 years, and the more capable surgeons have slowly retired, shouldn't surgical outcomes be getting worse?

Why do surgical outcomes across pretty much all procedures keep getting better over time if they're being increasingly performed by young incapable surgeons?

A few NPs have a Doctorate. That 1st NP Degree can be obtained with a 20 month online program. You can't get a MD online. 3rd and 4th years of Medical School are all Hospital/Clinical Training.

A NP can be a Cardiologist on Friday and a Gastroenterologist on Monday.

So to say that even a NP with a Doctorate is even close to a 1st year Resident is far from being honest.

And 85% reimbursement for an NP Note not Cosigned by a Doctor? That's the biggest lie of all.

Being clear is kind.

I was in the operating room the other day, during a stressful part of a case I needed a moment of complete focus, so I asked everyone in the room to be quiet. A few minutes later, after we had moved through that part of the operation, I apologized to the anesthesiologist.

He looked at me and said, "Being unclear is unkind. Being clear is kind."

No one has ever said that to me before.

I've always believed that honesty and direct communication are acts of respect, especially at work and in healthcare. Patients deserve clarity. Teams deserve clarity. In the operating room, where every decision matters, clarity creates trust and keeps patients safe.

I know I can be intense.

My intensity has always been about caring deeply. It's about protecting my patients. It's about making sure everyone in the room knows exactly what I need so we can do our best work together.

What meant so much to me about his comment wasn't just the words themselves. It was that he understood what was behind them.

He didn't see intensity. He saw kindness.

I couldn't ask for more than to work with people who understand that, and to be that kind of surgeon for my patients.

Theres confusion why hitting a 35-year record for measles is a bad thing. To be clear, 2,318 cases means:
-545 hospitalized
-12,300 school days missed
-Lose immunity to other viruses
-2 kids w permanent hearing loss
-1 kid will die in 10 years from brain disease
-$204m costs
