1/5: Our work in the TOP trial is now in JAMA. TOP randomized 1428 high cardiac risk patients after major general or vascular surgery to a restrictive (Hb < 7 g/dL) or liberal (Hb < 10 g/dL) transfusion strategy. 👇

Kicking off Chief Resident Grand Rounds with Dr. Tim Le’s presentation “Surgical Style.” 🩺👔 After intro by Dr. Hashmeet Kaur, Dr. Le delivered a phenomenal talk on navigating the challenges of residency by being affable, available, and able. Congratulations Dr. Le! 👏 @KougiasP
Next up—Dr. Vince Dodson’s Grand Rounds presentation “Surgery Residency: What Obstacles?” Introduced by Dr. Evelyn Ross, Dr. Dodson presented an inspiring talk incorporating Camus and Kierkegaard on redefining success in residency and life. Congrats Dr. Dodson! 👏 @KougiasP

@farkomd Agree. In addition, there is so many different types of bias that can sneak in a database study of this type that the results become difficult - if not impossible - to interpret
@TCTMD Congratulations to Dr. Panos Kougias (@KougiasP), Dr. Sherene Sharath (@ShereneSharath), and all the TOP trial investigators and coordinators!
An important medical advance made possible by @VAResearch and the participation of 1,400+ Veterans.

4/5: It also underscores the research imperative to clarify the effect of transfusion strategy on heart failure and arrhythmias, & the need to investigate patient populations that may benefit from liberal strategies using sophisticated monitoring and diagnostic techniques.👇
@AHAScience 💬 Editorial: While restrictive transfusion remains standard, TOP trial results indicate some cardiac patients may benefit from a tailored approach based on clinical factors.

Liberal transfusion after major surgery did not reduce death or major cardiac event rates compared to restrictive thresholds in high-risk patients. 🧵
