Could this be the path out of the employer-sponsored model? If ICHRAs are being spent on state exchanges, it swings the arm even further toward public(ish) coverage
Fascinating to hear about ICHRAs and their use on the marketplace exchanges. Essentially an employer-sponsored stipend (in place of traditional employer-sponsored insurance) for employees to spend on their own plans.

This episode features Jakob Emerson, Associate News Director at Becker's Healthcare. Here, he discusses CMS finalizing a rule to streamline the prior authorization process and improve the electronic exchange of health information & more.
Regarding the stipulation of no inpatient services:
I get it from an efficiency and dollars standpoint. But there has to be processes and support ($) in place for transferring/upgrading pts
Tough on pt support systems, who will have to travel further to visit and see pt
Reading up on the Rural Emergency Hospital program.
Extra $ for rural hospitals (strict criteria) if they offer ED +outpt services but *no* inpatient services

Sacubitril/valsartan results in comparable benefit to valsartan alone in select pts with HFpEF regardless of disease duration

Nice @medpagetoday op-ed on the rise of Urgent Care centers, the void that they seek to fill, and why primary care and EDs should do more to fill that void

On the topic of triaging -
This is a major point of failure of the American acute care apparatus. We can't expect patients to know how serious their symptoms are and where they should present. On top of embedded cultural, SES factors...

The erosion of the physician-patient relationship over time has almost certainly contributed to increased mistrust of the system
At some point, is greater trust in "average" care better than less trust in "exceptional" care?
β¬οΈ this is one of the reasons why having MDs in leadership roles can be so effective. You don't just get the clinical experience, but also a guarantee (usually) of mission and values
"People in some of the best academic health centers in the world or the best health systems in the world are saying that they don't necessarily have what they need to actually take care of patients," SCAN Group CEO @sacjai says. βThis is a very tough time to be in health care.β
