Oncological outcomes of deferred systemic therapy for patients with metastatic renal cell carcinoma: A systematic review and quantitative analysis

This systematic review suggests that active surveillance or other deferred systemic therapy strategies are viable and safe options for carefully selected patients with metastatic renal cell carcinoma, particularly those with favorable prognostic features. Across 1⃣5⃣ studies including nearly 3,000 patients, approximately half of patients avoided systemic therapy for up to three years without compromising overall survival, which was comparable to outcomes with immediate treatment. Favorable Rini risk scores were associated with better outcomes, supporting patient selection as a key factor.
Although the evidence is limited by study heterogeneity, selection bias, and reliance on tyrosine kinase inhibitor–era data, these findings highlight the potential of personalized, observation-based approaches to reduce overtreatment while maintaining oncologic efficacy in mRCC. #KidneyCancer
