Inlexo exhibits a higher initial CR, but it tends to decline more rapidly with a greater incidence of higher-grade AE, which are already significantly worse (13% compared to 2-5%). The higher initial CR for Inlexo is due to genetic testing, selecting only 1 in 5 patients, so real-world results may differ. As with all chemotherapy, Inlexo's CR will also decrease faster over time, leading to a higher recurrence rate.

A concern with Inlexo is that the device must be inserted every three weeks by a trained healthcare provider, with each procedure posing risks like infection, often requiring prophylactic antibiotics. Prolonged antibiotic use presents additional serious risks.

In contrast, Anktiva + BCG provides more durable disease control, with CR declining more slowly. The 12-month CR for this combination is 58%, compared to 52.4% for Inlexo. The full picture will become clearer once 24-month CR data for Inlexo is available.

For years we thought immune exhaustion was a slow, months-long process in cancer patients. Newer data shows T cell and NK cell exhaustion can happen in hours. That changes the entire timeline of when intervention matters. IL-15 is one of the few signals we have that can genuinely reverse that exhaustion once it's set in, which is why the delivery question matters as much as the drug itself.

Anktiva in lung cancer: $IBRX advanced the completion date of the Quilt 2.023 phase 3 trial by 14 months, likely securing sufficient long-term data and a solid foundation to support the confirmatory randomized phase 3 ResQ201A trial of Anktiva plus CPI. Such data could support a BLA in the US, similar to the approval already granted in Saudi Arabia.
