58/M with anterior STEMI, VF and cardiogenic shock. Now post LAD PCI and impella placement. Systolic blood pressure 100-110 post pci on Impella/ Norepi. 100 mL contrast used. Poll below. #cardiacarrest

@mandeep_mayo If it’s a STEMI I might try, if ivus crosses definitely will. By default always predil to see if it yields. Pro would be less chance of thrombus embolization. Con would be chances of this being fibrotic causing underexpansion. Lithotripsy saves the day tho!

Did exactly what @ShariqShamimMD had suggested. Guide extender into mRCA then wire with balloon support. I think lack of calcium helped!

Distal delivery of IC nicardipine/ nipride using twin pass or pronto while preparing for IV imaging!
