One of the biggest failures I see defensively is understanding your operator lineup and the utility you bring. Plenty of defenses can be won strictly by altering utility based off attacker positioning. As an IGL and/or a coach you should be thinking about what happened in each and every round: what utility did they bring, where did they come from, what can I change in my strategy in order to better attack or defend against it. Strategies are guidelines, they’re not set in stone. Mix it up sometimes.
