Currently, in early 2026, there are slightly more than 100,000 people on the national transplant waiting list.
The waitlist is heavily skewed toward one organ, kidney. Because kidney disease is so prevalent (and because patients can survive on dialysis while they wait), kidneys make up nearly 90,000 of the people on that list.
For context, Transmedics transplant volumes reached 5,100 in 2025 and they have grown transplant volumes at a CAGR of ~70% from 2022 to 2025 without kidney. Thats why, with the introduction of Kidney OCS, management is confident they can double transplants in just two years to more than 20,000 from 2028 to 2030.
While international expansion and next-gen Heart and Lung OCS programs is expected to provide incremental upside, the sheer scale of the kidney market remains the definitive catalyst. For this reason, the remainder of the year may very well be bumpy as i) the international rollout itself may not be straight line and ii) the NTM analyst estimates wont consider the kidney opportunity yet.
In my opinion, any turbulence would be an opportunity to look through the noise.

As of today, flights are trending ~13.5% ahead of January. This would result in ~815 March flights and a Q1 total of ~2,130.
