Agree @maiadorsett This is the key difference in Advance Care Planning tools. #POLST is key for EMS as the majority of POLST appropriate patients lack capacity and CANNOT make decisions “in the moment” in a crisis.
An incredibly important point: A POLST form is NOT an Advance Directive.
Advance Directive: making plans in case of unexpected deterioration.
POLST form: A set of MEDICAL ORDERS that is written after a guided conversation with their doc (or APP).
