Its gets really technical as to when and where applied.
Ex: surcharge is not technically allowed on a Debit cd - but on CC.
Visa rule change = only actual 3rd pty costs are allowed to be passed.

@fiatjaf I never looked upon it as individual real time settlement.
In my payments architectural designs that use lightning, I always look at it as settlement between aggregation points. In that use case, lightning will be a game changer.
