Loosening of syndesmotic screws is a relatively common complication in the treatment of syndesmotic ankle injuries.

It can range between 20% and 40%.

Early identification and proper management of loosening is essential to ensure optimal functional recovery of the ankle.

Classically we assess signs of loosening on x-rays.

However, this case demonstrates that the diagnosis can also be reached with dynamic US.

We can identify hypermobility of the screw, absolutely abnormal findings.

Maneuver: tibio-fibular compression.
