Key clue: posterior acoustic shadowing. Gallstones are typically mobile and shadowing; polyps are generally wall-adherent, fixed, and nonshadowing.
A gallstone confirms cholelithiasis—not necessarily acute cholecystitis.

This apical 4-chamber echo demonstrates McConnell’s sign: RV free wall hypokinesis with relative apical sparing, raising suspicion for acute pulmonary embolism in the appropriate clinical setting.
CT confirmed a large bilateral central PE with right heart strain, and the patient ultimately underwent catheter-based pulmonary embolectomy.
