SLIPPING RIB SYNDROME
Dynamic ultrasound allows musculoskeletal structures to be evaluated in motion, reproducing the patient’s symptoms and revealing abnormalities that may be missed on static imaging. Its greatest value lies in demonstrating instability, subluxation, and mechanical conflict in real time.
This case shows a beautiful example of slipping rib syndrome. At the medial portion of the 11th floating rib, at the level of the chondrocostal arch, I performed a modified hooking maneuver by manually translating the floating rib over the adjacent 10th rib. Dynamic ultrasound demonstrated marked rib displacement, with the 11th rib overriding the 10th rib, while reproducing the patient’s typical symptoms.
The combination of dynamic rib subluxation and symptom reproduction provided the key findings needed to establish the diagnosis.

An interesting issue to consider in #calf pain and injuries in either professional or amateur athletes is an abnormal proliferation of adaptive veins, particularly in the transitional region between the deep posterior compartment and the superficial posterior compartment.

As I ponder on the milk bottle of treatment effects, and recent papers on placebo and contextual effects, I think we are possibly falling into the mereological fallacy. The whole treatment effect is not a simple addition of the nonspecific and specific effects. A brief thread 🧵

To close or not close the capsule?
At 2-years, 38.9% of patients had unhealed hip capsules after interportal capsulotomy without repair.
Despite unhealed capsules, overall outcomes (pain relief & function) were not significantly different between healed vs. unhealed groups.
@carlespedret Bit like flying: seatbelts on at take-off and landing, no seatbelts when at cruising speed unless you experience sudden turbulence. More risk of disaster during take-off, landing and sudden (directional and speed) changes! 😄
