@Derek_Leong_RPh @FralickMike @dunbar37 Sounds simplistic, but frankly I ask the patient what they are comfortable with. Most never want be admitted to hospital again, so it’s an easy decision. Then I make sure not to make them feel they are missing out by not restarting it.

Need to go beyond sick day discussion, and give pts “permission” to not take the med at all it it doesn’t fit
