@learnneurorad Looks like a Cav Mal. Depends on if symptomatic or not. If no symptoms, leave it alone. If symptomatic ( likely epileptogenic in the region) , MR SWI or GRE is most sensitive then surgical resection is recommended especially if extra lesional hemorrhage is present.
