IIRC I handled it like exhaustion - the more crazy you get exposed to (without magical help or time to work on your issues), the more obvious and disturbing the effects.
And I think I tried to tie whichever demon lord’s thematic flavor of madness best fit each PC.

1) A space-aroace-goblin, time-machine mechanic ❤���

2) PCs cloning themselves a thousand times, with it being balanced, and also... *horrifying*
