I had been trying to quietly disengage from this conversation (I promise!), but this pulled back in. Respectfully, I would suggest that not operating on biopsy proven NSCLC after SBRT should not be promoted as SOC. Several points important to make here:

"Bombshell" may be a stretch, but interesting to ponder long term results of JCOG0802. No meaningful differences between segmentectomy & lobectomy. Choice may ultimately come down to individual patient risk factors, tumor characteristics, & preferences.
