Another insurance/specialty pharmacy issue that makes no sense.

Script for injections every 4 weeks. PA approved. Specialty pharmacy will only fill the script exactly on the 28th day and not a few days early. Thus it gets to the patient a few days late every month.

I completed a P2P today for a patient with VEOIBD that is not responding to adalimumab and switching to Stelara. The P2P wanted to know why I was only asking for 1 infusion and wanted to know my long term plan after the infusion. This system is so broken.

Solution to get Adult GI doctors to do proactive TDM for anti-TNFs= take away their ability to use other biologics (like in peds bc not fda approved) than they will be forced to improve their ability to optimize anti-TNFs
