He barely dodged fire launched in close range, only noticed the lightning because Sasuke let him which gave him prep time, & he's not a sensor so he wouldn't notice Sasuke a mile+ away. He needed Nagato to find Kabuto.

Furthermore, he admitted he found Kabuto due to Nagato. Itachi isn't a sensor. Nothing suggest he would spot Sasuke or be suspicious.

He was even hit. Again, the thunder didn't even catch his attention until he saw Sasuke staring at it.

Your best bet is him noticing the Dragon Fire a mile+ away, but even when launched below him he barely noticed in time to dodge.
