@InspGadgetBlogs There's no way he suddenly developed a MH crisis at 6.00 and an hour later had killed himself and 3 others an hour later.
This would have been building up a long while.
Needs investigating, yes but to blame the last point of contact is harsh.
@Melissa_suze @juneslater17 I'm not saying that poor practice didn't happen, more that practice wasn't malicious.
Most nurses did what they could. The idea that a deliberate intent to murder vulnerable was occurring is aborrehent and I'll happily stand and say it
Where a complaint is valid, it must be heard
@Melissa_suze @juneslater17 So the only feasible explanation is a syringe driver, but even 5mg/ml is 12ml and you are going to want to dilute it to reduce irritation. What size syringe are community drivers using these days?
@Melissa_suze @juneslater17 It's a slightly different situation of course because you are using these drugs to promote better oxygenation by optimising the breathing, so not necessarily symptom relief for end of life.
@Melissa_suze @juneslater17 As a sub cut infusion, you should only be achieving 2.4 mg/HR which is tolerable dose, bearing in mind this titrated over a period of time.
No one is starting at 60 mg / 24 hr / SC
@Melissa_suze @juneslater17 5mg/1mg or 2mg/ml are the only other strengths in the UK but availablity is restricted since Jan 2018 to acute specialist areas.
Certainly not community setting or general wards.
@Melissa_suze @juneslater17 I doubt even in hospital anyone got to a 60mg infusion!
Interesting point though, you didn't use the pump unless they where unconscious? Why wait that long, symptom relief needs to commence earlier doesn't it?
@Melissa_suze @juneslater17 Of course the 60mg dose is not really practical is it?
Certainly not in the community setting where you have 1mg/ml dosing you'd need a pump capable of taking 50 ml syringe.
@UniqueGav @FlowerXCm @yay_its_james Staff who would have been off duty as well, because staggering though it may seem, other staff would have been working with the patients!!!
