💧 Fluid Management in Sepsis and Septic Shock
Early, but Not Excessive
This core chapter revisits sepsis resuscitation in the era of stewardship, showing how initial fluids remain essential to reverse life‑threatening hypoperfusion, but fixed large bolus protocols can lead to harmful volume overload when applied indiscriminately. It integrates the ROSE concept, emphasizing short, targeted resuscitation followed by optimization and, where needed, timely evacuation.
Dynamic Assessment and Early Vasopressors
The authors advocate dynamic tests of fluid responsiveness (passive leg raising, stroke volume variation, echocardiographic indices) and early vasopressor support rather than repeated blind boluses. Evidence linking positive fluid balance with mortality in sepsis is summarized, underlining the need for explicit stop rules and deresuscitation plans once shock has resolved.
📘 Full sepsis algorithms and case vignettes in Chapter 14.
🔵 IFAD 2026 in Antwerp will devote multiple sessions to sepsis fluid management, including hands‑on hemodynamics and stewardship.

🩺 More than 500,000 people across the EU are receiving kidney replacement therapy (KRT) – a figure that has grown by around 150% since 1990.
The latest ERA Registry data highlights both the advances made in treating kidney failure and the challenges that remain.
Key findings include:
📌 Wide variation in KRT prevalence and treatment patterns across the EU
📌 Haemodialysis remains the predominant treatment pathway for patients starting KRT
📌 Survival outcomes differ markedly between treatment modalities, with transplantation associated with better long-term outcomes
As the number of people living with kidney failure continues to rise, strengthening early CKD detection, ensuring equitable access to KRT and reducing disparities in care across Europe will be critical to improving patient outcomes.

▶️Is there any true evidence in this area?
Measuring pre and post-HD BUN, creatinine, and electrolytes is an evidence free area
Certainty in evidence -GRADE approach to access quality of evidence. Based on the quality if data and publications, RCTs and observational studies can be ⬆️or ⬇️ graded.
BUT clinical judgment and expertise along with patients' values and understanding shape clinical practice. Effective health care also has to be cost-effective.
