The "KISS" sign spells "SICK" in the reflection in the mirror, and ironically, it's positioned above Maria's head.
She also has a squint eye and a droopy mouth while waiting in Heaven's Night. That's possibly a memory of Mary's face while she was sick, as she has a squint eye in the Leave ending, where she's seen sick.
James might be having flashbacks of Mary in her deathbed while they're in Heaven's Night, making Maria's face briefly overlap with Mary's.

When Simon first transitions to the Otherworld, he suddenly finds himself in a tiny empty room with a light above him, just like James when he transitions to the Otherworld in the hospital.

Mary's body is in the backseat of James' car, here's an off-camera look.
In the Stillness ending, he hallucinates that her dead body has come back to life.
He killed her a few days ago and has since kept her in the car, according to the novel. He tries to convince himself she died 3 years ago, but in fact she was diagnosed 3 years ago, and that's when she "died" to him.

If you look closely, the blanket she's covered with is stained in some areas, possibly because Mary is in early decomposition and her bodily fluis are seeping through.
