The claim that the Ugly bill will not close rural hospitals is misleading. Here's why:

HR 1, §71401 does establish a "Rural Health Transformation Program" with $10 billion/year from 2026–2030 to support struggling rural hospitals.

BUT.

What's left out?
Severe Medicaid cuts undermine rural hospitals.
§71112–§71120 impose work requirements, cost-sharing mandates, and spending restrictions on Medicaid - rural hospitals rely heavily on Medicaid reimbursements. These cuts are expected to reduce rural hospital revenues drastically.

Payments to rural hospitals are phased down (§§71118, 71120, and Medicare payment alignment rules).
➡️$10 billion/year for 5 years = $50 billion total (§71401)
➡️ $50B in aid vs $1T in cuts - that's just 5 cents returned for every $1 taken.

States cannot use their own funds to make up the shortfall (§71401(h)(2)(ii)) - even if local governments want to save a hospital, they're banned from filling the gap.

No appeals. No judicial review.
If CMS says a state misused funds, the money is reclaimed with no recourse (§71401(h)(8)).
Under §71401(h)(1)–(8), the HHS Secretary - under Trump - has sole discretion to approve, revoke, or reclaim rural hospital funding.
§100001–§100005, gives the President sweeping authority to direct, override, or reorganize executive agencies - including HHS. So, Trump can decide who gets help - and who doesn't. No appeals. No judicial review.

So, there's money for rural health on paper.
But it's paired with:
- brutal Medicaid cuts,
- bans on state intervention,
- and federal clawback power with no review.
That's not protection. It's a ticking time bomb for rural care systems.
The rural health "funding" is a PR cover for a massive system drain.

Don’t fall for the straw man argument that the Big Ugly Bill will only throw people off Medicaid who don’t meet work requirements. Work requirements just mean more paperwork and confusion and more cost to the states. Repubs want to trip people up then blame them for falling.
