In his last few months Biden had to deal with the Israeli attack, two trips abroad where he caught covid and he was actually sick during the debate. He walked different because of a sprained ankle. He didn't fall asleep in meetings.

The 2 weeks before that disastrous debate he went on 2 diplomatic trips abroad where he caught covid and twisted an ankle. He was ill during the debate and not rested from travel.
