How much of today’s note actually reflects what happened today?
With copied-forward documentation, template overuse, and note bloat, finding the true story of a patient encounter can feel like searching for a needle in a haystack. When documentation becomes cluttered, it becomes harder to support medical necessity, defend coding decisions, and maintain compliance.
In this episode of the CodeCast Podcast, Terry Fletcher breaks down the challenges of documentation overload and shares practical insights on creating clearer, more accurate records that tell the patient’s story.

Can physicians and clinicians use text messaging for patient care? 📱
While CMS and The Joint Commission do not prohibit texting orders, there are important HIPAA privacy and security considerations healthcare organizations must understand before implementing it into patient care workflows.
In this episode, Terry Fletcher breaks down compliance requirements, safeguards organizations should have in place, and a newly accepted AMA CPT Committee decision for 2027 that could impact future coding and reporting.

I’ve been screaming about this since 2020. How could any parent, physician, teacher or any adult subject or encourage a child to destroy their body and future?
Claire Abernathy was put on testosterone at 14, had a double mastectomy before she was in high school, and was fighting to have her medical transition reversed by 17. Now 21, she is detransitioning and urging lawmakers to help protect minors. https://t.co/kqvD3XtJx1
Today, we pause to remember the brave men and women who made the ultimate sacrifice for our freedom.
Memorial Day is more than a long weekend. It is a time to honor the heroes who gave everything in service to our country and to reflect on the courage, sacrifice, and dedication they carried with them every day.
To all the families who carry the weight of that sacrifice, we remember and honor your loved ones today and always.

HHS has released updated Notice of Privacy Practices (NPP) requirements for 2026, and this is more than just a template update.
These changes impact healthcare providers, health plans, and Part 2 patient notices involving sensitive PHI, including substance use disorders, reproductive health information, and mental health records.
While many think “NPP” means Non-Physician Practitioner, in this case it refers to patient privacy notice requirements that MUST be updated by July 1, 2026. The rule became effective February 16, 2026, and organizations that haven’t reviewed their notices yet could already be behind.
In this episode, Terry Fletcher breaks down what practices need to know, what’s changing, and why these updates matter for HIPAA compliance in 2026.

When states like California refuse to work with the federal government on saving American taxpayer dollars, @CMSGov and the @WHFraudTF will hold those states accountable.
Today, CMS notified the California Dept. of Health Care Services that we are deferring $1.3 billion in federal Medicaid funding:
https://t.co/15CHj8eesJ
.@VP: "We're announcing that the federal government is deferring $1.3 billion in Medicaid reimbursements from the state of California — and the simple reason is because the state of California has not taken fraud very seriously."
🚨 JUST NOW: Dr. Oz confirms there is $100 BILLION of Medicare and Medicaid fraud
"We would be able to DOUBLE the life expectancy of the Medicare Trust Fund if we could deal with the fraud issues just in Medicare!"
"If these fraudsters are willing to take your money, they will definitely steal your life and take your health along the way."
Telehealth modifier 95 and audio-only modifier 93 are under the microscope, and the denial trends are telling a bigger story. 👀
In this episode of the CodeCast Podcast, Terry Fletcher breaks down the claims data behind high-volume CPT codes, modifier usage, and place of service combinations that are triggering denials and audit concerns. From incorrect POS reporting to documentation gaps and risky billing habits, Terry covers what providers and coders need to know now.
She also tackles a growing issue in healthcare coding: relying on coding forums for “final” coding advice and why that can quickly turn into a compliance problem.

CMS doesn’t define a strict “12-month rule”… but that doesn’t mean you’re safe.
If a physician hasn’t seen the patient in 12–24 months, “incident to” billing could raise audit risk.
Terry breaks down the gray areas and how to protect your practice.

"They dishonestly claim people are dying when they’re not," says @DrOzCMS on hospice fraud in California.
"We’ve already delisted 450 hospices. That is an unbelievably large number—and guess what...no one's calling us saying 'No, no, we’re innocent.' They know they got caught."
Terry breaks down a recent social media post that exposed a practice’s compliance issues with time based care management services, drawing exactly the kind of attention no organization wants. Although the original post was quickly deleted after backlash, the situation highlights a bigger issue.
In this episode, Terry emphasizes the importance of using sound judgment online and ensuring time based services are properly documented and driven by medical necessity, not simply billed because they can be.

There’s something about the ocean that just resets everything, isn’t there?
Today is a reminder that the same world we rely on every day is worth protecting, appreciating, and taking care of, little by little, choice by choice. Whether it’s stepping outside for a breath of fresh air, reducing waste, or simply being more mindful… it all adds up.
Happy Earth Day 💙 Let’s take care of the place that takes care of us.

50-STATE AUDIT: The Trump administration will require all 50 states to explain their plans to revalidate some of their Medicaid providers in a national escalation of anti-fraud efforts that have so far largely focused on specific states.
Modifier 59 is under the microscope in 2026, and misuse is leading to audits, denials, and even recoupments tied to medical necessity
If you’re still second-guessing when to use it (or not use it), this episode is one you don’t want to skip.
On today’s CodeCast Podcast, Terry Fletcher breaks down proper Modifier 59 usage, the common mistakes that trigger audits, and the documentation you actually need to stay compliant. She also dives into post-op pain injections, when they’re reportable and who can bill for them.

I genuinely don’t care if two thirds of both houses of congress have to be expelled. I want as many like Swalwell, Gonzalez, Lamonica McIver, Sheila McCormick, Adam Schiff, Ilhan Omar, etc, as there are out. I don’t want them censured. I want them out of our government. I also want them investigated and, if the evidence warrants, indicted and prosecuted. Our government has become not just a geriatric home for people like Schumer, McConnell, and Pelosi, but also a den of criminals who believe they are above the law. So far, they have been. It is my hope and prayer that is about to change.
