Today we’ve submitted our very first referral for Discharge Medicine Service via PharmOutcomes. This is a huge achievement and will greatly support continuation of care once our patients have been discharged #DischargeMedicinesService #VirtualWard @RotherhamNHS_FT @RotherhamCTD
