A new kid on the block of large-effect rare variant associations: loss of function variants in BSN (a brain-specific gene) increase BMI by an effect size that is even larger than that of MC4R mutations (which cause monogenic obesity). Amazing!
Analyzing the UK Biobank exomes to capture those rare variants with extraordinarily large effect sizes sometimes feels like hunting for magical beasts in an enchanted forest. What is invisible through one type of binocular becomes visible through another.
Analyzing ~420k exomes from the UKB, the authors found that heterozygous protein truncating variants (PTVs) in BSN increase BMI by ~3kg/m2. BSN PTV carriers are at
- 6.6 fold higher risk for severe obesity
- 3.7 fold higher risk for NAFLD
- 3-fold higher risk for type 2 diabetes.
Interestingly, the BSN variants have no association with childhood adiposity. Thanks to proteomics data in the UK Biobank, overlapping the GWAS variants and BSN pQTLs, the authors also find a non-coding common variant signal 200kb upstream of BSN.
BSN encodes for a neuronal scaffolding protein called Bassoon that seems to be essential for presynaptic cytoskeleton organization and neurotransmitter release. Mice lacking Bsn seem to show lower excitatory neurotransmission in the brain and BSN missense variants have been linked to neurodegeneration in humans.
BSN mutations seem to cause a type of adult-onset obesity through a yet-to-be-understood neural mechanism. Whatever that might be, I am sure it will turn out absolutely fascinating.
