I saw the knot analogy referred to again this week in regard to complex systems and it reminded me of this reflection on complex discharge. The role of our IDFs being to assist in simplification and untangling the knot to find a pathway forward
We were delighted to have a student Occupational Therapist join us today to learn about the role of our discharge hub. Our service supports the whole MDT to coordinate effective and timely discharge plans. Happy to have students from all disciplines spend time learning with us π‘

Starting July there will be the reintroduction of a IDF working Sundays. This will allow us to capture the Monday discharges and review any complex discharge cases on site.
