Thyroid tumor with enlarged nuclei showing chromatin clearance, pseudoinclusions and grooves. Classic BRAF-like nuclear atypia.
Only problem - this is RET mutated medullary thyroid carcinoma.
Take home message: Be careful with MTCs, they may mimic anything.

Look out for a NICE Guideline 'Thyroid cancer: Assessment and Management' on 19th Dec. An earlier draft version in June '22 had various recommendations for pathology, cytology & thyroid MDT's so publication of the final version of this guideline document is awaited with interest!
@AshishC97225686 Yes, Grace had a true enthusiasm and interest in developments in her areas of practice, here ND rate for US guided thyroid FNA was ~1%. We had many emails over the years since I first met her at the 1stTBS meeting in Bethesda.
