To paraphrase Deming, when a team fails, look at what management has done to force the failure. Blame is never good, but if you're looking for causes, look up.
change consultants, agile coaches, or otherwise external 'change experts' don't "lead change". the people who will live with the consequences of the change do. our job is to support them.

The basic premise of Agile development is that predictability is impossible. Since you can't predict anything reliably, it's better to be adaptable than predictable. Plan accordingly. The desire for predictability is waterfall planning. Ppl don't understand how radical Agile is.

1/3 IME, large orgs pretty much ignore the concept of self organization entirely. They tell the teams what process/framework to use, they mandate things like Sprint length and ceremony timing (they almost always impose Scrum), they set deadlines, they push stories onto the teams,
The problems with all frameworks (e.g. Scrum) is that they codify something that some team developed for themselves, and which were effective for *that* team precisely because *that* team developed it. To be equally successful, *your* team needs to develop its own process.
When I’m tired I think life would be easier w/ top down hierarchy that people don’t like but accept as a necessary evil.
Creating a different type of org chart w/ autonomy and responsibility is no peace of cake.
But at the end of the day I want be proud of what we/I create!
