Between June 2025 and June 2026, 82% of Martha’s Rule calls relating to acute deterioration would not have triggered escalation based on the early warning score alone.
Yet within this group:
➡️ 13% resulted in a transfer of care
➡️ 56% resulted in another change in treatment
“But the ECG is normal…”
That sentence should never be the end of the assessment.
An ECG is a snapshot of cardiac electrical activity at one moment in time.
→ Reassess the patient
→ Repeat the ECG
→ Compare it directly with the previous trace
→ Look for dynamic changes
Good leadership doesn’t just create calm. It creates the conditions for others to perform.
An empowered team doesn’t just perform better when things are going well.
It helps bring the whole team from frazzle back into flow when things aren’t 🙏

Observations provide data.
Clinicians provide interpretation.
- A blood pressure of 110 mmHg doesn’t exclude shock
- An SpO₂ of 98% doesn’t guarantee adequate ventilation
- A normal HR doesn’t rule out serious illness
Our duty, is to ask ‘Why?’
A reassuring SpO₂ does not always mean a reassuring patient.
• Respiratory rate, depth and pattern
• Chest movement and air entry
• Work of breathing, fatigue
• Consciousness
• ETCO₂
Sometimes the most dangerous deterioration is the one hidden behind a normal number 🚩
The more experienced we become, the easier it is to stop questioning what we know.
Experience gives us pattern recognition. It makes us faster, more confident and more decisive. But those same strengths can quietly become vulnerabilities.
'Could I be wrong?'
“What physiological problem am I trying to solve with PEEP, and what might I be compromising by using it?”
Every ventilator setting has a consequence.
