The Dec 13 deadline they gave us has passed. Healthcare continuity matters - we need clarity on how to retrieve our records to transition to new providers.

@goforward just announced immediate closure of all locations & their app. As a patient needing continuity of care, key questions remain:
- What happened to cause this sudden shutdown?
- Will patient records be automatically transferred?
- Are there partner clinics taking on Forward patients?
Any additional info would help your community navigate this transition.
