Wish Bear didn't throw up and overdosed because he actually loved Nikki and knew that his death would free her

He knows in reality she's suffering because of his wish, and if he's anything like wish Nikki, he'd sooner die than hurt her.

So he lets himself OD as an act of love. In that moment he does the one thing he knows will make the real Nikki happy, and that's to free her
