Intranuclear inclusions are created by redundancy of nuclear membrane leading to invagination/protrusion of the cell cytoplasm through the nucleus. This INCI from a cystic subtype of Papillary Thyroid CA proves that fact and even has the cytoplasmic vacuoles seen within it. (FNA)

Despite the low power appearance of a lymphocytic background with oncocytic-appearing epithelium, the presence of high N/C ratio cells with macronucleoli argue against the dx of a Warthin tumor. An AR+ IHC confirmed the final FNA diagnosis of "Salivary Duct Carcinoma". (Parotid)

"Multinucleated giant histiocytes" although quite eye-catching, have no real pathologic significance in FNAs of Papillary Thyroid Carcinoma. This one, turned out to be a "tall cell" subtype.
