Now, ask yourself what it portends for Naval's definitions of "functioning" and "merit-based" (in the context of merit being mutually exclusive of participation, which he implies).
@SethyPoker I voted $200, but this is a dumb scenario to say "double or nothing" to, because it's effectively the same thing as saying, "go again?" and the latter is clearer.
@TseLabs@Jimbohdtu@mcuban I'm well aware that goes into the cost of A, as do a number of other vital overhead costs. The problem is that this information is not itemized and transparent in a way where we can discuss it other than conceptually. That's a problem.

A) The lowest possible cost the service could be provided at ("cost plus" some minimally necessary margin)
B) What the hospital charges for it
The spread between A & B is often enormous, but invisible to consumers.
