biggest changes:

- exchanges auto-summarize to ~300 chars (78% fewer tokens in context)
- old bloated memories get cleaned up automatically, 1 per exchange
- retrieval fairness — memory_bank no longer dominates every search
- unused memories now drift toward cleanup instead of sitting forever
