Gastroparesis ≠ just delayed gastric emptying.

Great talk today by #AmolSharma on looking beyond the gastric emptying study and thinking more carefully about the phenotype in front of us.

A few clinical pearls I’m taking home:

🔹 Nausea ≠ vomiting. Understanding the predominant symptom matters when choosing therapy.

🔹 Gastroparesis may be a pan-enteric disorder - constipation is common, correlates with symptom severity, and deserves attention.

🔹 A gastric emptying study is only as good as the protocol used to perform it. Standardization matters.

🔹 The risk of tardive dyskinesia with metoclopramide may be considerably lower than historically quoted, while still requiring thoughtful patient selection and monitoring.

🔹 Prokinetics, antiemetics, neuromodulators, GES, and pylorus-directed therapies such as G-POEM each have a role - but not for every patient with an abnormal gastric emptying study.

My biggest takeaway:

Treat the patient’s phenotype, not the gastric emptying number.

The farther we move beyond simply asking “Is gastric emptying delayed?” toward asking “What is actually driving this patient’s symptoms?” the more thoughtful and individualized our treatment becomes.
