Why not Ceftriaxone?

Need Ceftriaxone for intra-ab infxns - when narrower spectrum, less ESBL/VRE selecting, and less C diff causing Amp-sulbactam works just as well for PNA bugs, use that.

@ABsteward Why shouldn't be cefoxitin be used as an empirical anaerobic infection therapy when dosed appropriately covers about 95% of the isolates? I'd rather cover Pseudomomas spp in most cases when I'm covering anaerobic bacteria but that's bein beyond the scope of the matter.

@DavidHollandMD Doxycycline, like most antimicrobials, shouldn't be used lightly; it's unethical to compromise its future activity because of indiscriminate abuse, besides, it's not *that* effective in this indication. I'm totally down for PrEP but this is different.
