The X-rays of the hands and feet showed lytic lesions that completely destroyed the bone of the distal phalanx of the third finger and great toe. This, in the context of malignancy, is in keeping with acrometastases.

Acrometastases are a rare type of bone metastasis that occur in <0.1% of tumours and involve sites distal to the elbow or knee. This finding should prompt a search for a primary malignancy.

Gout can cause a similar clinical appearance, but tends to affect joints whereas bone is affected here and can be differentiated with plain radiographs.
