Hawaii’s Medicaid Fraud Control Unit has been getting about three million dollars a year in federal funding for over a decade. But between 2022 and 2025, they had zero indictments and zero criminal convictions for Medicaid fraud or patient abuse, even as enrollment jumped.

The Trump administration just cut that funding on June 4th because of it. The state’s Attorney General says they’ve recovered about fourteen million through civil settlements since 2021 and filed a couple charges recently, but the feds weren’t satisfied with the lack of criminal prosecutions.
Governor Josh Green launched a new state-funded strike force to fight it, and they’re appealing the decision. This is all about provider fraud.

Most of the Medicaid fraud in Hawaii is provider-side, doctors, clinics, home health agencies, nursing homes, and equipment suppliers.
Common types include:
• Billing for services never provided, that’s the most basic and common one.
• Upcoding, charging for a more expensive service than what was actually done.
• Duplicate billing, unbundling, billing for stuff that’s not medically necessary.
• Diverting or misusing drugs.
The unit also handles patient abuse and neglect in care facilities. They don’t usually go after beneficiaries unless they’re working with a crooked provider.
Beyond Medicaid, there’s the usual stuff, unemployment insurance fraud, workers’ comp scams, and some private health insurance fraud.
