In the new Townfall gameplay trailer Simon unlocks a door by cutting an incision on a patch of skin. That's a reference to the monsters that attack him with a needle through an incision in their chest.
I'm sure the monsters are Simon. Their height and body type seems to be the same as Simon's, and they have an S carved in their faces. If he's indeed in a coma like I've said in previous posts, these "monsters" might be what he looks like in the real world. Their thin arms and loose skin are common among people who have been bedridden for a long time, because of muscle mass loss. If he has lost his hair in the real world like the monsters, it could be either from a disease such as cancer, or because his body is going through extreme medical stress, which can happen in patients who are in a coma.
