“We did our best with the staff and resources available, but it wasn’t enough.” That is the seed of a moral injury. @ProfNGreenberg@bmj_latest Recommended reading today for the weeks ahead when grappling with myriad doubts of being 'good enough' as individuals and systems
A key paper from @ProfNGreenberg et al. setting out measures that healthcare managers need to put in place
to protect the mental health of healthcare staff having to make morally challenging decisions during the Covid-19 pandemic https://t.co/8YdFmxmP6E
@nuwandiss@ProfLAppleby The challenge will be that even if AOT is reintroduced widely (which I hope it will be)…that it must be resourced to deliver for all who meet the criteria, rather than a caseload cap. The college have been unable to provide guidance on caseloads for cmht.
@nuwandiss@wendyburn@AgnesAyton thank you for your helpful testimony Diss . Some of your opinions resonate with the challenges faced by those working community mental health teams. I do hope further consideration will arise around timely information sharing between key agencies &, training re: risk of violence
We have been recommissioned to study patient homicide having not done so since 2018. Key messages from our previous studies highlight that addressing comorbid alcohol/drug misuse & delivering deliver the care plan as intended is crucial to reduce risk #NCISH2026@ProfLAppleby
Some of you on here will have known Dr Tony Zigmond who sadly died yesterday. He worked for many years as a psychiatrist in Leeds and was an Hon Fellow and former Vice President of @rcpsych. He will be greatly missed. RIP Tony.
@asifamhp But then they'll bring in the s2/s3 amendments and there will be fewer admissions - all part of the master plan to detain fewer and fewer people.
It's expensive you know - easier and cheaper to push costs to CJ system and just promise that lessons will be learned.
In fact, consultant psychiatrists have in reality also been displaced from leadership of community pathways. They are only nominally consultant led. Key pathway decisions no longer necessarily rest with consultants. Admitting rights removed.

@NeilDotObrien Excellent piece - there is a conversation nobody has been wanting to have about why the police and Crown Prosecution Service are reluctant to prosecute mentally unwell people who commit more serious offences; and how initiatives like "Right Care, Right Person" contribute to that.
GP referrals will now be ‘assessed’ by a ‘Consultant led’ Single point of access
1 in 4 is the target to reject
GPs & patients must insist on Jess’s Rule in referrals
Any rejected referral must have a Consultant name & GMC number
This is the only way to ensure patient choice
This is a photograph of Albert Einstein with an unassuming Indian man you probably haven’t heard enough about. He spent his life working on one idea: women should be able to live with dignity and make their own choices. Thread.

"Hour by hour, the fire spread through the space between the arch’s inner and outer layer, which was designed to keep a hermetic seal. Four days after the strike, it was completely ablaze. Worse, the fire had spread deep into an interior membrane made of polymers and other materials — wrapped all around the arch — that firefighters could not reach. There was only one way to fight the fire, and it involved cutting even more holes in the arch." ...
Read this terrifying and well presented @FT story about the hole punched in Chornobyl's rooftop by a Russian drone that the @EBRD says will cost €500 to fix.

Thora Hird's performance in Alan Bennett's Talking Heads is always singled out, and quite right too. But Patricia Routledge's performance as Irene Ruddock in A Lady of Letters breaks your heart. I've never forgotten this scene.
Patricia was an incredible dramatic actress.
Use of 'insight' in medicolegal settings to help in decisions about involuntary treatment and decision-making capacity. Conclusion: can be useful if done in the right way and contra NICE, insight and capacity are NOT 'distinct concepts'
